Opening new cases by visiting the client's home, gather and document personal and medical information.
Assessing living conditions.
Assessing the client's needs.
Reassessing clients every 60 days providing one in person visit and the others being phone visits. At that time the LPN will document any changes in the client's health, behavior, home and needs.
