The Nurse Navigator provides coordination and support for patients with heart failure across the continuum of care. This role focuses on assessing patient needs, delivering education, and coordinating care plans with providers and interdisciplinary teams to ensure timely, high-quality outcomes.
The Nurse Navigator supports disease management through patient education, follow-up, and care coordination, helping to improve outcomes and reduce hospital readmissions. Responsibilities include monitoring patient progress, facilitating transitions of care, and ensuring adherence to treatment plans and best practices.
This position plays a key role in promoting a seamless, patient-centered care experience while supporting quality, safety, and operational effectiveness within the Heart Failure Clinic. All responsibilities are performed in alignment with Mercy’s mission, values, and Service Standards.

Nurse Navigator – Heart Failure Clinic (PRN)


Mercy Hospital South

Supports patients with heart failure and their families throughout evaluation, treatment, and ongoing management, ensuring a seamless outpatient experience.

Coordinates individualized, evidence-based care plans in collaboration with providers and interdisciplinary teams to optimize outcomes and reduce hospital readmissions. Serves as a clinical resource for physicians, staff, and patients, supporting best practices in heart failure management.

Identifies and manages patient populations through defined clinical criteria, tracking outcomes and quality metrics to support improved care delivery. Actively participates in process and quality improvement initiatives focused on patient safety, efficiency, and outcomes.

Ensures accurate and timely documentation in EPIC and other systems to support continuity of care and data-driven decision-making. Partners with providers and care teams to enhance patient access, education, and engagement, while supporting program growth and community awareness.

Promotes a team-focused environment with a commitment to professional growth, collaboration, and continuous improvement.

Qualifications

Education: Graduate from an approved school of nursing
Licensure: Current RN license in the state of practice
Experience: 2–3 years of nursing experience required
Certifications: BLS required
Preferred Education: Bachelor of Science in Nursing (BSN)
Preferred Certification: ACLS

Physical Requirements

  • Ability to push, pull, and/or lift up to 50 pounds
  • Prolonged standing and walking throughout the shift
  • Ability to bend, reach, kneel, twist, and squat as needed
