RN Care Manager - Compact License

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical needs.



Key Responsibilities

Evaluate member needs and develop personalized care plans
Coordinate care between members, families, and healthcare providers
Monitor member progress and update care plans as necessary


Required Qualifications

Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
2 - 4 years of related experience
Registered Nurse (RN) state licensure or Compact State Licensure
Behavioral health experience preferred
Location in the eastern time zone is required
