Transition Specialist

A company is looking for a Transition Specialist to support the coordination of member transitions in the community.



Key Responsibilities

Support care coordination teams to develop effective transition plans for members
Assist with member transitions based on enrollment or care service changes
Track and maintain transition metrics to identify trends and process improvements


Required Qualifications

Bachelor's degree required
2 - 4 years of related experience preferred
Experience in children's division or inpatient hospital settings preferred
Equivalent experience may be considered
Knowledge of care coordination and transition services is beneficial
