Review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability. Provide information and advice regarding drug interactions, side effects, dosage, and proper medication storage. Collaborate with other health care professionals to plan, monitor, review, or evaluate the quality or effectiveness of drugs or drug regimens, providing advice on drug applications or characteristics. Ensure safe, appropriate, and cost-effective medication therapies in accordance with established clinical guidelines, approved treatment protocols, and internal patient care policies. Serve as a resource for unbiased drug information and education related to pharmaceutical care, drug therapy, and medication therapy management for providers, caregivers, staff members, and patients. Assist providers and chronic disease managers in developing and managing complex drug regimens and measuring clinical outcomes. Assist the Manager of Clinical Pharmacy Services in reviewing formulary products to support effective pharmaceutical stewardship for the patient population. Perform medication reconciliation assessments as needed for hospital, emergency room, or urgent care discharges, and as requested by clinic providers. Review and monitor medication therapy regimens for allergies, adverse drug reactions, drug drug interactions, therapeutic duplications, contraindications, and inappropriate dosing. Develop workflows, protocols, and standards of practice for medication reviews conducted by staff pharmacists. Develop medication therapy assessment tools for staff pharmacists when a clinical pharmacist is unavailable for consultation. Maintain professional competence and complete continuing education as required for the position. Provide direct patient care services through home or facility visits, if requested by providers or Care Coordination. Provide immunization services.
Active State Board-issued Pharmacist License
Doctor of Pharmacy (Pharm.D.) degree
